Travel Home Health Registered Nurse job duties

  • Conduct comprehensive assessments of patients' health status, needs, and environment during home visits.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
  • Administer medications, treatments, and injections as prescribed by physicians, ensuring proper documentation and adherence to protocols.
  • Monitor patients' vital signs and overall health, recognizing and responding to changes in condition promptly.
  • Provide wound care, including dressing changes, infection prevention, and monitoring for signs of complications.
  • Educate patients and their families on disease management, medication administration, and lifestyle modifications to promote health and independence.
  • Assist with activities of daily living, such as bathing, grooming, and mobility, to support patients' comfort and well-being.
  • Coordinate with other healthcare professionals, including physical therapists, occupational therapists, and social workers, to ensure comprehensive care.
  • Document all patient interactions, assessments, interventions, and outcomes accurately and timely in electronic health records.
  • Provide emotional support and counseling to patients and their families, helping them cope with illness, injury, or disability.
  • Ensure compliance with healthcare regulations, policies, and ethical standards in all aspects of home health care.
  • Participate in ongoing professional development and continuing education to stay current with advancements in home health nursing practices.
  • Promote patient safety by conducting home safety assessments and providing recommendations to reduce risks.
  • Maintain open communication with patients, families, and the healthcare team to ensure effective care coordination and patient satisfaction.
