Physical Therapist - Outpatient - (PT)
StartDate: 12/16/2024 Available Shifts: 8 D Pay Rate: $2163.34 - $2492.55

PT needed asap for 13 weeks Monday - Friday 8am-5pm The Physical Therapist I/II / Advanced Physical Therapist I/II position serves to provide quality physical therapy services to a predominately Native American population for the treatment or prevention of physical disability or disease. ** Fingerprinting must be submitted and cleared prior to start. ** REQUIRED: Bachelor’s degree from the Commission of Accreditation in Physical Therapy Education (CAPTE) accredited school of Physical Therapy. Must possess a valid, current, full and unrestricted license in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico during tenure of employment. Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. Applicant must meet requirements to be credentialed to serve on the Medical Staff. All related information required for credentialing must be provided. ORIENTATION: Every two (2) weeks only. COVID EXEMPTIONS: We do not accept religious exemptions. If a candidate submits for a medical exemption, we will have to get approval from our medical review board to proceed with onboarding. HOUSING APPLICATION: must be submitted 2 weeks prior to start date - form uploaded in Medefis

Required Qualifications
Physical Therapist, Acute Experience: 3 years Licenses: PT(Compact Accepted) Certifications: BLS-AHA(Copy needed) AND Physical Therapist, Outpatient Experience: 3 years SSN Required DOB Required ** Fingerprinting must be submitted and cleared prior to start. **
