o Minimum of 2 years experience - Required o Cover page - Note start, RTO, Specialty, any specific info traveler would like client to know o Work History o Skills Checklist (pertinent to position), current to 1 year o 2 Supervisory Signed References (from within the past two years of employment) o License/Certifications (as required by posting: NH RN, BLS, ACLS o BLS (AHA) - Discipline/Specialty: Physical Therapist (PT) - Setting/Unit: Outpatient Rehabilitation - Start Date: - Contract Length: 13 Weeks - Shift/Schedule: 8hr Days - Weekly Hours: 40 - Minimum Experience Required: MINIMUM 2 years post grad experience - Registration/Certification Required: Copy of BLS (AHA), Copy of Covid Card or Vax proof, Copy of NH License verification or PT Compact
