Physical Therapist - Home Health - (PT - HH)
StartDate: 4/21/2025 Available Shifts: 8 D Pay Rate: $1819.54 - $1993.75

Illinois licensed PT for Home Health duties in New Lenox. 8a-4:30p Mon-Fri with 40 hours/week guaranteed. BLS, 1 year Home Health expereience required. Covid vax (no declinations) and valid drivers license/insurance required.

Required Qualifications
Years of Experience: 1. Illinois state license; BLS
1 year Home Health PT experience - REQUIRED
Valid drivers license and car insurance - REQUIRED
COVID vaccination required - DECLINATIONS WILL NOT BE APPROVED **Please add vaccination status in the profile**
