The facility is seeking experienced RN Case Managers for a 13-week contract. This role is essential in ensuring seamless patient care transitions and discharge planning within an acute care hospital setting.
Active CA RN license
BLS/BCLS certification
At least 2 years of RN experience
Strong recent experience in discharge planning
Recent acute care hospital experience
familiarity with EPIC EMR for documentation
