Home Health RN - Registered Nurse with MSSI in Minnetonka, MN starting ASAP - 2x12 hr days - $2085.00/week

Nurse Consultant - 30 hours per week minimum but could work up to 40 hours per week. MN RN license and AHA BLS required. Must be able to travel within a 2-hour radius of main office. Must have at least 2 years nursing experience in the following areas: home care, mental health, community-based nursing, skilled nursing facility, public health, visiting nursing, and/or private duty nursing. Must have at least 1 year experience working with underserved populations such as those on the Medicaid waiver program or other socio-economically challenged populations. Main roles will involve educating patients, care team, and caregivers in sub-specialty of medication therapy adherence, vitals tracking/monitoring, and improving client health. Candidate will also review alerts, follows up with patient/clients, creates/implements care plans and goals to improve and maintain client health and independence. Candidate must be comfortable working with clients and their care givers in their homes with multiple, complex, and chronic health conditions including behavioral health populations. Looking for a candidate that is technologically savvy, have computer proficiency (Microsoft Office Suite, Google Suite, etc.) and EHR/EMR experience. Prefer candidates to have critical thinking/problem solving skills, flexibility and open to assisting growth and change in organization.  This key role is performed in a hybrid environment. Nurse Consultants work primarily in client?s private homes/group home settings and virtually (either from home if a private office space is available or at our main office). Work may also need to be conducted in your vehicle or other location while traveling. Candidates must have reliable transportation and enjoy a mix of working from home / office and traveling to multiple client home locations daily.
