Physical Therapist - Home Health - (PT - HH)
StartDate: 12/2/2024 Available Shifts: 8 D Pay Rate: $1924.09 - $2216.88

PT needed asap 40 hrs. per week, Rot Days, Occ. Weekends Occ. Holidays. Unpaid half hour Lunch EPIC exp preferred. prior home visit experience. Be able to Eval and PT care starts as well Bachelor of Science Degree or Masters Degree in Physical Therapy from an institution accredited by the Counsel on Medical Education of the American Medical Association and by the American Physical Therapy Association. Current PT license in Ma required. Current BLS certification required. Start of care as well as case management of a home based caseload of a variety of patients - up to 6 units each day Working in the Upper Cape - discussed during interview
